Arizona Healthcare Directives Registry is a state run portal where client’s advance care directives, and documents that outline a person’s healthcare preferences can be uploaded and accessible to medical professionals in Arizona. This allows for people’s end-of-life care wishes to be respected. Documents included in the portal are the following: Living Will, Health Care Power of Attorney, Mental Health Care Power of Attorney and Pre-hospital Medical Care Directive – Do Not Resuscitate (DNR).

As of January 2022, Hunt & Gale are partners of the Arizona Healthcare Directives Registry. This means, we can act as the agent for our clients who wish to participate. We will provide instructions and will assist you to. We upload our clients’ relevant documents to the portal as they are completed. You will be asked to confirm this choice in writing since you will be waiving the attorney-client privilege.

Portal Instructions

When we complete your health care directive documents, we will set up an account for you, and you will receive a letter or email, depending on which you choose. From there, you will be able to set up an account on AzHDR. If you already have an account, please access your account with the first section below.
